Lumbar Disc Herniation
A lumbar disc herniation occurs when disc material in the lower back irritates or compresses a nearby nerve. It may cause lower back pain, pain travelling down the leg, numbness, tingling or muscle weakness. Treatment decisions are based on the patient’s symptoms, neurological examination and imaging findings together.
MRI findings alone do not determine treatment
Disc herniations are commonly seen on MRI scans, including in people without significant symptoms. The examination findings and the way symptoms affect daily life are essential when deciding on treatment.
Lumbar disc herniation: the short answer
A lumbar disc herniation occurs when part of a disc in the lower spine bulges or moves beyond its normal boundary and irritates a nearby nerve. It can cause lower back pain, pain travelling into the buttock or leg, numbness, tingling or muscle weakness.
Most people improve without surgery through appropriate activity modification, medication and physiotherapy. Surgery may be considered when there is progressive muscle weakness, severe persistent nerve pain or symptoms that significantly restrict daily life despite non-surgical treatment. The MRI scan, symptoms and neurological examination must always be assessed together.
Symptoms That May Require Urgent Medical Assessment
Most lumbar disc herniations do not require emergency treatment. However, certain symptoms may indicate significant nerve compression or another serious underlying condition and should not be ignored.
- New or rapidly worsening muscle weakness in one or both legs
- Difficulty starting urination, inability to urinate or loss of bladder control
- Loss of bowel control
- New numbness around the genitals, inner thighs, buttocks or the area used when sitting on a saddle
- New weakness, numbness or severe pain affecting both legs
- New loss of genital sensation or sexual function
These symptoms may indicate cauda equina syndrome, a rare but serious condition caused by severe compression of the nerves at the base of the spine. Seek emergency medical care immediately if any of these symptoms develop.
Arrange Prompt Medical Assessment If You Have:
- Severe back pain following a fall, accident or other significant trauma
- Back pain accompanied by fever, chills or feeling generally unwell
- Unexplained weight loss
- Persistent pain that is severe at night or continues to worsen
- A history of cancer, significant infection or reduced immune function
- Pain or neurological symptoms that are rapidly worsening
These findings do not always mean that a serious condition is present. However, they require medical assessment to determine the cause and whether further investigation or urgent treatment is needed.

What Is a Lumbar Disc Herniation?
The lumbar spine is the lower part of the back. Between the lumbar vertebrae are flexible structures called intervertebral discs, which help absorb forces and allow the spine to move.
Each disc has a softer inner centre, called the nucleus pulposus, surrounded by a stronger outer layer, called the annulus fibrosus. A lumbar disc herniation occurs when part of the inner disc material moves through a weakened or torn area in the outer layer.
If the herniated disc material irritates or compresses a nearby nerve root, it may cause pain that travels from the lower back or buttock into the leg. This radiating nerve pain is commonly called sciatica. Numbness, tingling or muscle weakness may also develop depending on which nerve is affected.
A lumbar disc herniation does not always cause symptoms. Disc changes and herniations may be visible on an MRI scan in people who have little or no pain. For this reason, the MRI findings must be interpreted together with the patient’s symptoms and neurological examination.
Lumbar disc herniation is anatomically defined by displacement of inner disc material through the surrounding outer layer. Symptoms generally develop when this material irritates or compresses a nearby nerve root.
Does a “Slipped Disc” Actually Slip?
“Slipped disc” is a commonly used term for a disc herniation. However, the entire disc does not actually slip out of place. The term describes inner disc material moving beyond its normal boundary through a weakened area in the disc’s outer layer.
Is Lumbar Disc Herniation the Same as Loss of Lumbar Lordosis?
No. A lumbar disc herniation occurs when disc material in the lower back moves beyond its normal boundary and may irritate or compress a nearby nerve root. Loss of lumbar lordosis means that the normal inward curve of the lower back appears reduced or straighter.
Loss of lumbar lordosis does not by itself mean that there is a disc herniation. It may be reported on an X-ray or MRI because of muscle spasm, pain, posture during imaging or the person’s general spinal alignment.
Both findings may appear in the same imaging report, but treatment decisions should not be based on these terms alone. The pattern of pain, whether symptoms travel into the leg, the presence of numbness or weakness, the neurological examination and the imaging findings should be evaluated together.
What Are the Symptoms of a Lumbar Disc Herniation?
The symptoms of a lumbar disc herniation vary depending on the location of the herniated disc, the nerve root involved and the degree of nerve irritation or compression. Some people experience severe symptoms, while others have little or no discomfort.
The most characteristic symptom is pain that travels from the lower back or buttock into the leg. This is commonly called sciatica. The pain may feel sharp, burning or electric and may extend into the thigh, lower leg, foot or toes.
Nerve irritation may also cause numbness, tingling or muscle weakness in specific areas of the leg or foot. Symptoms usually affect one leg, although both legs may occasionally be involved.
Coughing, sneezing, straining, bending or prolonged sitting may temporarily increase leg pain in some patients. However, the pattern and severity of symptoms can differ considerably between individuals.
Common symptoms of lumbar disc herniation include pain travelling through the buttock and leg, numbness, tingling and muscle weakness. Not every disc herniation causes symptoms.

Is Lower Back Pain Alone a Sign of Lumbar Disc Herniation?
Not necessarily. Lower back pain is extremely common and may arise from several different structures and conditions. A lumbar disc herniation is more likely to be clinically significant when the pain travels into the leg or is accompanied by numbness, tingling, muscle weakness or examination findings consistent with nerve irritation.
An MRI finding of a disc herniation does not prove that it is the source of the patient’s pain. The location of the disc herniation should correspond with the symptoms and neurological examination findings.
Seek urgent medical assessment if symptoms are rapidly worsening or if you develop progressive weakness, numbness around the genital or saddle area, or changes in bladder or bowel control.
What Causes a Lumbar Disc Herniation?
A lumbar disc herniation usually develops because of a combination of changes within the disc and mechanical stress placed on the lower back. Over time, the outer layer of a disc may become less flexible and more vulnerable to small tears. Part of the softer inner disc material can then move through the weakened area.
Symptoms may begin gradually, without a clear triggering event. In other cases, pain starts suddenly after lifting, bending, twisting or another movement that places additional pressure on the disc. A single movement may trigger symptoms, but it is not always the sole cause of the disc herniation.
Lumbar disc herniation can also occur in active, healthy people. It does not necessarily mean that the patient has damaged their spine through carelessness or that they must permanently avoid normal movement and exercise.
Age-related disc changes are a common underlying factor. Improper lifting, repeated bending or twisting, physically demanding work and prolonged sitting may increase stress on the lumbar discs.
Factors That May Increase the Risk
- Natural changes in the discs over time
- Repeated heavy lifting, bending or twisting
- Lifting while the back is bent or rotated
- Physically demanding work
- Prolonged sitting or driving
- Low levels of regular physical activity
- Excess body weight
- Smoking
- Previous injury or repeated strain affecting the lower back
- Genetic susceptibility to disc degeneration or herniation
Smoking, excess body weight, repetitive physical work and genetic predisposition are recognised risk factors for disc herniation.
Does Lumbar Disc Herniation Always Result From Lifting Something Heavy?
No. Heavy lifting or an awkward movement may trigger the onset of symptoms, but many lumbar disc herniations develop without a specific injury. Changes within the disc often occur gradually before pain begins.
Identifying one exact movement as the cause is not always possible. The more important questions are which nerve is affected, whether there is muscle weakness and how much the symptoms restrict daily activities.
Can Lumbar Disc Herniation Be Completely Prevented?
Not every lumbar disc herniation can be prevented. However, remaining physically active, using appropriate lifting techniques, maintaining a healthy body weight and avoiding smoking may help reduce stress on the spinal discs and support general spinal health.
How Is a Lumbar Disc Herniation Diagnosed?
The diagnosis of a lumbar disc herniation begins with a detailed discussion of the patient’s symptoms and a physical examination. The location of the pain, whether it travels into the leg, the presence of numbness or weakness and the effect of symptoms on daily activities all provide important information.
During the examination, the doctor assesses muscle strength, sensation, reflexes and walking ability. Certain movements or nerve-stretching tests may also be used to determine whether a lumbar nerve root is irritated.
An MRI scan can show the spinal discs, nerve roots and surrounding soft tissues in detail. However, an MRI is not routinely required for every episode of lower back or leg pain. Imaging is most useful when the result is likely to change treatment, when symptoms persist or progress, or when significant nerve compression or another serious condition is suspected.
MRI findings must always be interpreted together with the patient’s symptoms and examination. A disc herniation seen on an MRI scan may not be responsible for the symptoms if its location does not correspond with the affected nerve.
Clinical history and neurological examination are central to diagnosis. NICE advises against routine imaging in non-specialist settings unless serious underlying disease is suspected, while MRI is generally appropriate when persistent or progressive symptoms may lead to an intervention or surgery.
The Assessment May Include:
- The location and pattern of lower back and leg pain
- Whether the pain extends below the knee or into the foot
- Numbness, tingling or altered sensation
- Muscle strength in the legs, ankles, feet and toes
- Tendon reflexes
- Walking on the heels or toes
- Straight leg raise or other nerve-stretching tests
- Bladder, bowel or saddle-area symptoms
- The effect of symptoms on sleep, work and daily activities
Which Imaging Tests May Be Used?
MRI Scan
MRI is the preferred imaging method when detailed assessment of the discs and nerve roots is required. It can show the level and position of a disc herniation and whether a nerve appears compressed.
X-Ray
An X-ray does not show a lumbar disc herniation directly. It may occasionally be used to assess the bones, spinal alignment or other possible causes of pain.
CT Scan
A CT scan may be considered in selected cases, particularly when MRI cannot be performed. It provides detailed images of the bones but generally shows discs and nerves less clearly than MRI.
MRI provides detailed soft-tissue images, whereas X-rays mainly assess bone and alignment. CT may be used selectively when MRI is unsuitable or unavailable.
Do I Need an MRI Immediately?
Usually not. Many patients with symptoms consistent with lumbar disc herniation can initially be assessed and treated without immediate imaging.
An early MRI may be required if there is progressive muscle weakness, suspected cauda equina syndrome, significant trauma, signs of infection or cancer, or another concerning finding. MRI may also be requested when symptoms persist despite appropriate treatment and surgery or an injection procedure is being considered.
For uncomplicated acute lower back pain or sciatica without red-flag findings, routine initial imaging is generally not recommended.
Does the Size of the Disc Herniation Determine the Severity of Symptoms?
Not always. A relatively small disc herniation may cause significant symptoms if it irritates a sensitive nerve root, while a larger herniation may cause few symptoms in another patient.
Treatment decisions should therefore not be based only on the reported size of the herniation. The patient’s pain, neurological findings, functional limitations and imaging results must be considered together.
What Are the Treatment Options for Lumbar Disc Herniation?
Treatment for a lumbar disc herniation is planned according to the patient’s symptoms, neurological examination, imaging findings and the effect of the condition on daily life. There is no single treatment method that is suitable for every patient.
Many patients improve without surgery. Initial treatment commonly focuses on controlling pain, maintaining appropriate activity and allowing the irritated nerve time to recover. Medication and a personalised physiotherapy or exercise programme may be recommended according to the patient’s needs.
If severe leg pain continues despite appropriate non-surgical treatment, an image-guided spinal injection may be considered for selected patients. An injection may help reduce nerve-related pain and support rehabilitation, but it does not physically remove the disc herniation.
Surgery may be considered when there is progressive muscle weakness, serious nerve compression or persistent leg pain that significantly restricts daily life despite appropriate non-surgical treatment. The decision to operate should not be based on the MRI scan alone.
Most people with a symptomatic slipped disc do not require surgery. NICE recommends considering spinal decompression when non-surgical treatment has not improved pain or function and the imaging findings correspond with the patient’s sciatic symptoms.
Treatment May Include:
- Monitoring symptoms and allowing time for natural recovery
- Remaining appropriately active
- Temporary modification of activities that worsen symptoms
- Medication selected according to the patient’s medical condition
- Physiotherapy and an individualised exercise programme
- Image-guided spinal injections for selected patients
- Surgery when clinically indicated
How Is the Most Appropriate Treatment Selected?
The MRI appearance is only one part of the assessment. The doctor also considers the severity and duration of pain, the presence of numbness or muscle weakness, the affected nerve, previous treatments and the degree to which symptoms restrict walking, working, sleeping and other daily activities.
Two patients with similar-looking MRI scans may therefore require different treatments. One may improve with non-surgical care, while another may benefit from an injection or surgery because of persistent disabling pain or neurological findings.
What Is the Aim of Treatment?
The main goals are to reduce pain, protect neurological function and help the patient return safely to normal daily activities. Treatment should address the patient’s symptoms and functional limitations rather than attempting to change an MRI image alone.
What Are the Non-Surgical Treatment Options?
Most patients with a lumbar disc herniation can initially be treated without surgery, provided that there is no progressive muscle weakness, cauda equina syndrome or another condition requiring urgent intervention.
Non-surgical treatment does not attempt to force the disc back into its original position. Its aims are to reduce pain, maintain neurological function, support the body’s natural recovery process and help the patient gradually return to normal activities.
Treatment should be tailored to the severity of symptoms, the patient’s general health, neurological examination findings and the activities affected by pain. Progress should be monitored, particularly when numbness or muscle weakness is present.
NICE recommends providing self-management advice, encouraging patients to continue normal activities and selecting exercise according to the individual’s needs and abilities.
Activity Modification and Gradual Return to Movement
During the most painful period, temporarily reducing activities that clearly worsen leg pain may be helpful. However, prolonged bed rest is generally not recommended.
Patients should remain as active as their symptoms safely allow and gradually return to walking, work and normal daily activities. Short and frequent periods of movement may initially be more comfortable than prolonged sitting, standing or walking.
Temporary activity modification does not mean permanently avoiding bending, lifting, exercise or other normal movements. Activities can usually be reintroduced gradually as pain and neurological symptoms improve.
The Literature advises that brief rest may sometimes be necessary when pain is severe, but gradual return to gentle exercise and normal activity generally supports recovery.
Medication
Medication may be used for a limited period to help control pain and allow the patient to remain active. The choice of medication should be based on the patient’s symptoms, age, existing medical conditions and other medicines being taken.
Anti-inflammatory medicines may be considered for some patients, but they are not suitable for everyone. Potential stomach, kidney, liver and cardiovascular risks should be assessed before they are used.
Medication should not be continued automatically if it provides little benefit. It should be reviewed if pain persists, side effects develop or stronger medication is being considered.
For sciatica, NICE advises that anti-inflammatory medicines have limited evidence of benefit and potential harms; when prescribed, the lowest effective dose should be used for the shortest possible period after individual risk assessment.
Are “Nerve Pain” Medicines Always Helpful for Sciatica?
Not necessarily. Medicines commonly described as nerve-pain treatments are not automatically effective for leg pain caused by lumbar disc herniation.
According to NICE guidance, gabapentinoids, other antiepileptic medicines, oral corticosteroids and benzodiazepines should not routinely be offered for sciatica because there is no overall evidence of benefit and there is evidence of harm. Opioids are also not recommended for chronic sciatica.
Patients already taking these medicines should not stop them suddenly without medical advice, as some require a supervised reduction plan.
These medication recommendations and the need to discuss withdrawal risks are specifically stated in NICE guidance for sciatica.
Physiotherapy and Exercise
Physiotherapy may help the patient remain active, restore confidence in movement and gradually improve strength, mobility and tolerance for daily activities.
An exercise programme should be adapted to the individual. Exercises that are appropriate for one patient may temporarily increase another patient’s leg pain. The programme may therefore be adjusted according to symptoms, neurological findings and recovery.
Physiotherapy should focus on active rehabilitation rather than relying only on passive treatments. Manual therapy may sometimes be used as part of a broader programme that also includes exercise.
NICE recommends exercise tailored to the patient’s needs and considers manual therapy only as part of a treatment package that includes exercise.
Monitoring and Reassessment
Symptoms should be reassessed if pain is not improving, daily activities remain severely restricted or numbness and weakness are increasing.
Persistent pain does not automatically mean that surgery is required. However, further examination or imaging may be appropriate when symptoms continue despite treatment or when an injection or surgical procedure is being considered.
Progressive muscle weakness, changes in bladder or bowel control, or numbness around the genital or saddle area require urgent medical assessment.
Does Non-Surgical Treatment Mean Simply Waiting?
No. Appropriate non-surgical treatment is an active process. It includes controlling symptoms, maintaining safe activity, monitoring neurological function and gradually restoring normal movement and daily activities.
The treatment plan should be reviewed if recovery does not progress as expected or if new neurological symptoms develop.
Treatments Not Routinely Recommended
Treatments that rely mainly on prolonged rest or passive modalities are unlikely to provide a complete solution. NICE does not recommend traction, belts or corsets, acupuncture, TENS, therapeutic ultrasound or interferential therapy for routine management of lower back pain with or without sciatica.
Any treatment should have a clear purpose, measurable benefit and a plan for helping the patient return to normal activity.
When May Spinal Injections Be Considered?
Spinal injections are not routinely required for every patient with a lumbar disc herniation. They may be considered for selected patients who have severe nerve-related leg pain, commonly called sciatica, that has not improved sufficiently with appropriate non-surgical treatment.
During an epidural injection or selective nerve-root injection, medication is delivered close to the irritated nerve. The injection commonly contains a local anaesthetic and a corticosteroid intended to reduce inflammation around the nerve and temporarily relieve pain.
The main purpose of an injection is to reduce leg pain sufficiently for the patient to remain active, sleep more comfortably and participate in rehabilitation. The degree and duration of pain relief vary between patients, and some patients may experience little or no improvement.
An injection does not physically remove the disc herniation and does not permanently repair the disc. It should be considered as one possible part of a broader treatment plan rather than a guaranteed cure.
NICE recommends considering epidural injections containing local anaesthetic and steroid for people with acute and severe sciatica, but advises against using spinal injections to manage non-specific lower back pain alone.
An Injection May Be Considered When:
- Severe leg pain is the patient’s main symptom
- Symptoms and examination findings suggest irritation of a specific nerve root
- MRI findings correspond with the patient’s symptoms
- Appropriate medication and activity modification have not provided sufficient relief
- Pain prevents the patient from participating effectively in physiotherapy or rehabilitation
- Temporary pain relief may help the patient return to normal activity
- A selective nerve-root injection may help clarify which nerve is causing symptoms in a carefully selected case
What Can a Spinal Injection Achieve?
A successful injection may temporarily reduce inflammation and pain around the affected nerve. This can create an opportunity for the patient to increase activity and progress with rehabilitation.
Pain relief may last for a short period or considerably longer, but the response cannot be predicted with certainty. An injection does not guarantee that surgery will be avoided, and the absence of improvement does not automatically mean that surgery is required.
NHS patient information emphasises that epidural injections are intended to reduce pain and support mobility or rehabilitation, but should not be regarded as a cure for spinal pain.
Does an Injection Shrink or Remove the Disc Herniation?
No. The injected medication does not mechanically remove the herniated disc material or return the disc to its original position.
Its primary aim is to reduce inflammation and pain around the affected nerve. Any later change in the size of the disc herniation occurs through the body’s natural processes rather than because the injection physically dissolves the disc.
When Should an Injection Not Delay Further Assessment?
A spinal injection should not delay urgent assessment or necessary surgery when there is progressive muscle weakness, cauda equina syndrome or another serious neurological condition.
Patients with rapidly worsening weakness, bladder or bowel dysfunction, or numbness around the genital or saddle area require urgent medical assessment rather than routine injection treatment.
What Are the Possible Risks and Side Effects?
Spinal injections are commonly performed procedures, but no medical procedure is completely risk-free. The potential benefits and risks should be discussed before treatment.
- Temporary soreness or bruising at the injection site
- A temporary increase in pain
- Temporary numbness or weakness caused by the local anaesthetic
- Facial flushing or other temporary steroid-related effects
- A temporary rise in blood glucose, particularly in patients with diabetes
- Bleeding or haematoma
- Infection
- Headache following accidental puncture of the membrane surrounding the nerves
- Allergic reaction to medication or contrast material
- Rare nerve injury or other serious neurological complications
Temporary pain aggravation, numbness or weakness may occur after an injection. Bleeding, infection, allergic reaction and nerve injury are uncommon but recognised risks.
Information to Share Before an Injection
Before the procedure, inform the treating team if you:
- Take anticoagulant or antiplatelet medication
- Have a bleeding or clotting disorder
- Have diabetes
- Have an active infection or feel unwell
- Have allergies to medicines or contrast material
- Are pregnant or may be pregnant
Blood-thinning medication must not be stopped without specific instructions from the responsible medical team.
Anticoagulant use, infection and diabetes can affect procedural planning, and corticosteroid injections may temporarily raise blood glucose.
Can Laser Treatment Be Used for Lumbar Disc Herniation?
The term “laser treatment” can refer to several different procedures. In the treatment of lumbar disc herniation, it usually refers to percutaneous laser disc decompression, also known as PLDD.
During this minimally invasive procedure, a needle is placed into the affected disc under imaging guidance. A thin optical fibre is then passed through the needle, and laser energy is used to vaporise a small amount of the soft material inside the disc. The aim is to reduce pressure within the disc and relieve irritation of the nearby nerve.
Percutaneous laser disc decompression is not suitable for every type of lumbar disc herniation. It may be considered only in carefully selected patients who have persistent severe leg pain despite appropriate non-surgical treatment and whose imaging shows a contained disc herniation.
It is generally not appropriate when disc material has extruded or separated from the disc, or when progressive muscle weakness, cauda equina syndrome or another condition requires direct surgical decompression.
NICE states that percutaneous intradiscal laser treatment should be limited to patients with severe pain that has not responded to conservative treatment, an intact but bulging disc on imaging and no neurological deficit requiring surgical decompression. During the procedure, laser energy vaporises part of the nucleus pulposus.
Who May Be Suitable for Laser Disc Decompression?
- Patients whose main symptom is persistent nerve-related leg pain
- Patients who have not improved sufficiently with appropriate non-surgical treatment
- Patients with a contained disc protrusion rather than an extruded or separated disc fragment
- Patients whose MRI findings correspond with their symptoms
- Patients without progressive neurological weakness requiring direct decompression
- Patients who understand that further treatment or surgery may still be required
Is Laser Treatment the Same as Endoscopic or Microsurgical Discectomy?
No. These procedures are different.
Percutaneous laser disc decompression reduces pressure inside a contained disc by vaporising a small amount of its inner material. It does not directly remove a large extruded disc fragment from around the nerve.
During microscopic or endoscopic discectomy, the surgeon directly identifies and removes the disc material compressing the nerve. The most appropriate technique therefore depends on the type and location of the disc herniation, the neurological findings and the patient’s symptoms.
Is Laser Treatment Always Better Because It Is Minimally Invasive?
No. A smaller incision or needle-based procedure does not automatically mean that it is the most effective treatment for every patient.
Laser disc decompression may provide meaningful pain relief in appropriately selected patients. However, the available studies include different patient groups and treatment techniques, and the long-term evidence remains less certain than for established surgical procedures.
Some patients improve after laser treatment, while others later require another procedure or surgical decompression. The treatment should therefore be selected according to the clinical findings rather than because the word “laser” sounds more advanced.
A 2026 systematic review found meaningful pain reduction and a low pooled complication rate in selected patients, but also found substantial variation between studies, moderate risk of bias and a need for stronger comparative research.
Can Surgery Still Be Required After Laser Treatment?
Yes. Laser treatment does not guarantee that surgery will be avoided.
In a randomised study involving carefully selected patients, a treatment strategy beginning with laser decompression produced similar overall outcomes to a strategy beginning with microdiscectomy. However, recovery was faster after conventional surgery, and more patients who initially received laser treatment later required an additional operation.
Patients considering laser treatment should therefore understand both its minimally invasive nature and the possibility that direct surgical decompression may still be needed.
At two-year follow-up in the randomised trial, overall outcomes were similar between the treatment strategies, but additional surgery was required more frequently after initial laser decompression.
What Are the Possible Risks?
Although percutaneous laser disc decompression is minimally invasive, it is not risk-free. Possible complications include infection, inflammation within the disc, thermal injury, nerve damage, recurrent disc protrusion and the need for further treatment or surgery.
Careful patient selection, accurate needle placement and appropriate procedural experience are essential.
NICE lists recognised or theoretical complications including disc infection, heat-related tissue damage, nerve injury, recurrent protrusion, loss of disc height and perineural scarring.
What About External Laser Therapy Used in Physiotherapy?
External low-level or high-intensity laser therapy applied through the skin is different from percutaneous laser disc decompression. It does not enter the disc or remove disc material.
Some patients may experience temporary pain relief from physical therapy modalities, but external laser therapy should not be presented as a treatment that removes or repairs a lumbar disc herniation.
When Is Surgery Needed for a Lumbar Disc Herniation?
Most people with a lumbar disc herniation do not require surgery. Surgery is considered when the expected benefit of relieving pressure on the affected nerve is greater than the risks of continuing non-surgical treatment or undergoing an operation.
In non-emergency situations, surgery may be considered when persistent leg pain or sciatica continues to significantly restrict walking, sleeping, working or other daily activities despite appropriate non-surgical treatment. The symptoms, neurological examination and MRI findings should correspond with the same affected nerve.
Surgery may be recommended more urgently when there is progressive or significant muscle weakness. Suspected cauda equina syndrome, particularly new bladder or bowel dysfunction or numbness around the saddle area, requires emergency assessment and may require urgent surgical decompression.
The decision to operate should not be based only on the size or appearance of the disc herniation on MRI. Some large herniations improve without surgery, while a smaller herniation may require treatment if it causes severe persistent symptoms or neurological loss.
NICE recommends considering spinal decompression when non-surgical treatment has not improved pain or function and the imaging findings are consistent with the patient’s sciatic symptoms. Cauda equina syndrome is treated as a surgical emergency.
Surgery May Be Considered When:
- Leg pain remains severe and disabling despite appropriate non-surgical treatment
- Pain continues to substantially restrict walking, sleep, work or daily activities
- Examination findings and MRI results identify compression of the same nerve root
- Muscle weakness is significant or progressively worsening
- Symptoms repeatedly return and substantially affect quality of life
- The patient understands the expected benefits, limitations and risks of surgery
When May Emergency Surgery Be Required?
Emergency assessment is required when symptoms suggest severe compression of the nerves at the base of the spine.
Seek emergency medical care immediately if you develop:
- New difficulty starting urination or inability to urinate
- Loss of bladder or bowel control
- New numbness around the genitals, buttocks, inner thighs or saddle area
- Rapidly worsening weakness affecting one or both legs
- Severe neurological symptoms affecting both legs
These symptoms may indicate cauda equina syndrome. Delayed treatment may increase the risk of permanent neurological impairment.
Compression of the cauda equina can cause bladder, bowel and lower-limb neurological dysfunction and requires emergency referral to spinal surgical services.
What Is the Aim of Surgery?
The main aim of surgery is to relieve pressure on the affected nerve by removing the disc material responsible for compression. This procedure is commonly called a lumbar discectomy or microdiscectomy.
Surgery is primarily intended to improve nerve-related leg pain, commonly called sciatica. Lower back pain may also improve, but improvement in back pain is generally less predictable.
Numbness and muscle weakness may recover gradually after surgery, but complete recovery cannot be guaranteed. Recovery depends partly on the severity and duration of nerve compression and whether permanent nerve damage has already occurred.
Lumbar microdiscectomy removes the disc material compressing a nerve root. Its main purpose is nerve decompression, and improvement in leg pain is generally more predictable than improvement in lower back pain.
Is the Entire Disc Removed During Surgery?
Usually not. During a lumbar discectomy, the surgeon removes the disc fragment or material compressing the nerve. The entire intervertebral disc is not routinely removed.
Because part of the disc remains in place, another fragment can occasionally herniate from the same disc in the future. This is known as recurrent disc herniation.
During microdiscectomy, the portion pressing on the nerve is removed rather than the entire disc, so recurrent herniation can occur later.
Which Surgical Techniques May Be Used?
Microdiscectomy
Microdiscectomy is a commonly used operation for lumbar disc herniation. The surgeon reaches the affected nerve through an incision in the lower back and removes the disc material causing compression, often using magnification.
Endoscopic Discectomy
In selected patients, an endoscope and specialised instruments may be used to remove the herniated disc material through a smaller access route. Endoscopic surgery has the same primary goal as microdiscectomy: directly relieving pressure on the affected nerve.
Open Decompression or Additional Procedures
A wider decompression may occasionally be required when disc herniation is accompanied by spinal canal narrowing or other structural problems. The most appropriate approach depends on the location of the herniation, the anatomy of the spine and the surgeon’s assessment.
Open, microscopic and endoscopic approaches differ in their access route, but their central objective is to remove pressure from the compressed nerve.
Is Minimally Invasive or Endoscopic Surgery Always Better?
Not necessarily. A smaller incision may reduce tissue disruption in appropriately selected patients, but it does not automatically make a procedure safer or more effective for every type of disc herniation.
The most important goal is adequate and safe decompression of the affected nerve. The surgical technique should be selected according to the position and type of disc herniation, the patient’s anatomy, neurological findings and the surgeon’s experience.
Does a Large Disc Herniation Always Require Surgery?
No. The size of a disc herniation alone does not determine whether surgery is required.
Disc herniations can decrease in size over time, and many patients improve without surgery. However, surgery may still be appropriate when pain remains disabling, neurological weakness progresses or serious nerve compression is present.
Treatment decisions should therefore be based on the patient’s symptoms, neurological examination, functional limitations and imaging findings together.
NASS notes that most patients improve and that disc herniations often shrink or regress over time, although recovery varies between individuals.
How Long Should a Patient Wait Before Considering Surgery?
There is no single waiting period that is appropriate for every patient.
Patients without serious neurological findings are commonly given time for non-surgical treatment and natural recovery. Surgery may be discussed sooner when pain is intolerable or daily function is severely restricted. Progressive weakness or symptoms suggesting cauda equina syndrome should not be managed by simply waiting.
The timing of surgery should be decided individually after considering symptom severity, neurological findings, response to treatment and the patient’s preferences.
What Are the Possible Risks of Surgery?
Lumbar disc surgery is commonly performed, but every operation has potential risks. Before surgery, the expected benefits and possible complications should be discussed according to the patient’s individual condition.
- Infection
- Bleeding or blood clot formation
- Leakage of cerebrospinal fluid caused by a tear in the covering around the nerves
- Nerve-root injury or worsening neurological symptoms
- Persistent pain, numbness or weakness
- Recurrent disc herniation
- Scar tissue around the nerve
- Anaesthesia-related complications
- The need for further surgery
Recognised complications of lumbar decompression include cerebrospinal-fluid leakage, nerve injury, persistent symptoms and the possible need for another operation. Serious complications are uncommon but possible.
Does Lumbar Disc Herniation Surgery Require Spinal Fusion?
Usually not. An isolated lumbar disc herniation is generally treated by removing the disc material compressing the nerve without fusing the vertebrae.
Spinal fusion may be considered only when another significant structural problem, such as instability, deformity or a separate indication, is also present. It should not be assumed to be a routine part of disc herniation surgery.

Lumbar disc herniation is frequently associated with misleading or overly simplified information. Understanding what MRI findings, pain relief and surgery actually mean can help patients make more informed treatment decisions.
Does an MRI Showing a Disc Herniation Mean Surgery Is Always Necessary?
False: Seeing a lumbar disc herniation on an MRI scan means that surgery is always required.
Correct: An MRI finding alone is not sufficient to make a decision about surgery. Treatment should be planned by considering the patient’s symptoms, examination findings, signs of nerve compression and the effect on daily life together.
Is Prolonged Bed Rest Recommended for a Lumbar Disc Herniation?
False: Everyone with a lumbar disc herniation should remain on prolonged bed rest.
Correct: Prolonged bed rest is generally not recommended. Activities that clearly worsen pain may need to be temporarily limited, but maintaining controlled and safe movement is more appropriate for most patients.
If the Pain Improves, Has the Disc Herniation Completely Disappeared?
